Mo' Glow Loyalty Membership Agreement

This Membership Agreement ("Agreement") is entered into by and between Soul Beauty LLC, DBA Mo Beauty Aesthetics and Wellness and/or Vitality Collective ("Provider"), and the undersigned individual ("Member").

1. Membership Details:

• Membership Level: Mo' Glow Loyalty Program

• Monthly Cost: $149/month

• Carry Over E-Wallet Dollars: Yes, up to 9 months.

• Initial Term: Minimum 4-month commitment from the date of signing.

• Renewal Term: Automatically renews monthly unless terminated as outlined below.

2. Membership Benefits:

You are entitled to the following benefits as a Mo' Glow Loyalty Member. Discounts are applied to our published rates. A copy of published rates can be requested via email or when you are in the shop. Changes to our published rates can be made at the provider’s discretion with a 30-day written notice to members. 

• $2 off all Botox/Xeomin/Dysport per unit

• $100 OFF for any Dermal filler treatments

• 10% off Single Session Treatments (PRP, Laser, Microneedling, etc.)

• 10% off Medical Weight Loss Peptides

• 15% off Package Sessions (Like 3 Microneedling Sessions, Hydrafacials, etc.) 

Additional Perks:

• 10% off Zo Skin Health Products and Supplements * Some exclusions may apply

• 20-minute annual skincare consultations

• Access to exclusive Mo' Glow membership wellness events and model calls

• Priority booking for appointments

• You will receive a $25 referral reward for each friend you refer who joins our Loyalty program. This amount will be credited to your E-Wallet for a service or retail product.

• Birthday gift: Free Vitamin B-12 Shot or $25 off Zo Skin Health products.

4. Payment Terms:

• Membership fees are charged monthly using the payment method on file.

• An active credit card must be kept on file at all times.

• Late payments may result in a suspension or termination of membership benefits.

5. Scheduling and Cancellations:

• Unused E-Wallet Credits may roll over up to 9 months.

• Cancellations to your membership must be made, in writing, 30 days or more before the next billing cycle to avoid being charged for the subsequent quarter. Send emails to laurie@lovebrighter.com.

• If a Member cancels before the end of the initial 4-month term, they will be charged an early termination fee of $99.

• Appointment cancellations or rescheduling require 24-hour notice. Failure to provide adequate notice may result in forfeiture of the scheduled service and appropriated E-Wallet dollars.

6. Membership Termination:

• Members may terminate their membership after the initial 4-month term with 30 days written notice before the next monthly billing cycle. Send emails to laurie@lovebrighter.com.

• The provider reserves the right to terminate the membership if the terms of this agreement are not met.

7. Miscellaneous:

• Loyalty membership benefits are non-transferable and may not be shared.

• Provider reserves the right to modify benefits, services, or pricing with 30 days' prior written notice. Changes will be posted online, via email, or within the Aesthetic Record Membership Portal.

• Members agree to use the Aesthetics Record Membership Portal to manage their accounts and book future appointments (after the conversion to the new portal in Feb 2025).
